Management and Long-term Outcomes of Mild Cervical Spondylotic Myelopathy: a Systematic Review and Meta Analysis

Cervical spondylotic myelopathy can be mild but still could cause subtle signs and symptoms which includes hand weakness, sensory problem and poor coordination. The management of such cases aims to relieve or prevent progression of symptoms. Management is conservative that includes physical therapy, lifestyle modifications, medications and some would necessitate surgery. This study followed the PRISMA Guidelines and reviewed published articles from 2016 to 2023 that involved determined the management and outcomes of mild CSM. The search was conducted in MEDLINE via Pubmed, Cochrane Central Register of Controlled Trials, Herding Plus, Embase, and Google Scholar. Patient demographics, neurologic outcome, and clinical and imaging predictors were examined. We assessed study quality using the Newcastle-Ottawa Scale (NOS) for observational studies. We reported statistical data as presented and calculated RRs or ORs if not provided. Evidence quality was evaluated using the GRADE approach. Nine studies were included in this study. Isometric exercises emerged as a consistently effective intervention for alleviating neck pain and improving neck disability in patients with this condition. The findings suggest that the inclusion of isometric exercises in treatment plans could be a valuable approach for managing symptoms. The use of certain analgesics, specifically ropivacaine, had a potential in reducing pain among patients with cervical spondylotic myelopathy. Good outcomes have been observed with isometric exercises for the amelioration of neck pain and enhancement of neck disability, it is recommended that patients diagnosed with cervical spondylotic myelopathy incorporate these exercises into their rehabilitation regimens. It is advisable for healthcare practitioners to contemplate the prescription of individualized isometric exercise regimens in accordance with the specific physical conditions and requirements of each patient. Consistent evaluation and assessment of the exercises' efficacy ought to constitute an essential component of the treatment regimen.
