This is a prospective single center study from may 2024 to 2025 with minimum follow up of 1 year. Patients were operated in lateral position by transpsoas retroperitoneal approach under neuro Monitorning. Preop mri and x rays and post op mri and x rays were done taken at immediate post op,6 months post op and 1 year. Clinical assessment was done by VAS and oswertt disability index. Fusion was assessed by bridwells fusion grading. Total of 32 patients were analysed and parameters analysed are LL,SLA,ADH,PDH,FH with p value of <0.05 with improvement in clinical scores calculated both preop and post op. This study clearly states that DLIF with MISS PPF attained statistically significant improvement in LL,SLA,ADH,PDH,FH with improvement in clinical parameters and good fusion rates.
