A Five-Day Revolution in Spine Surgery: My APSS Fellowship at UMMC
Abdullah Al Mamun Choudhury
National Institute of Traumatology & Orthopedic Rehabilitation (NITOR), Dhaka
The APSS fellowship at Universiti Malaya Medical Centre (UMMC) is renowned for deformity correction, yet a five-day duration initially seemed insufficient for meaningful learning. This fellowship report describes how a compressed, high-intensity exposure to a high-volume deformity center transformed clinical perspective and surgical approach, challenging the assumption that longer duration alone determines educational value. The five-day program (2-6 December) at UMMC, Kuala Lumpur, provided immersive exposure to the deformity correction practice. The structured program included clinical case discussions, operating theatre observation and assistance, interaction with medical students, and a fellowship dinner fostering collegial exchange. Over four operative days, alongside three APSS colleagues, the fellow observed and assisted in ten complex scoliosis surgeries, including idiopathic, congenital, Lenke type 5, and Lenke type 6 deformities. Key Learning Points are Crossbar measurement technique for intraoperative correction assessment, Two-surgeon approach for complex deformity cases, Selective use of monoaxial screws for enhanced correction, Specialized rod holders facilitating efficient manoeuvrability, Attention to T1 tilt and sagittal parameters in surgical decision-making, Systematic approach to upper instrumented vertebra selection and Integration of published literature with live surgical observation A five-day fellowship, initially viewed as a limitation, proved to be an intense crucible of learning that compressed years of specialized experience into a focused beam of insight. The UMMC experience demonstrated that immersive, high-quality exposure to a masterful surgical team can revolutionize practice regardless of duration. The fellow returns inspired, strategically focused, and eternally grateful for one of the most memorable and transformative experiences of a surgical career.
